| Ordering products through ACI requires a Online
Authorization Code. This number can be obtained by filling out and
submitting a "Terms Agreement" form. This form can be downloaded from the link
below. To complete the submission process, click the link
below to download and view the form. You may then print the form and fill it out or you
may fill it out online by typing in the blank areas. To submit the form to ACI, mail it to
the address at the bottom of the form or fax it to 870-850-8440.
When your form is received and accepted, you will receive you
customer order number in the mail. Use this number when purchasing products online with
ACI.
