Subj:  Revised Policy – Medicare Savings Reevaluations

Summary of Changes

Medical Services Policy 23160 has been revised to incorporate the use of the DCO-811, Medicare Savings Benefits for Qualified Medicare Beneficiaries Annual Renewal Notice, when reevaluating cases. This form replaces the use of the DCO-808 and DCO-777 for reevaluations. Form DCO-662 will be required at reevaluation only when there has been a change in insurance coverage.
