Summary of Changes
MS 3322.5 has been revised to clarify the procedure when the Medical Review Team (MRT) decision is that the individual does not meet the disability requirements and Social Security Administration (SSA) later determines the individual does meet the disability requirements for the same time period.
MS 3323.2 has been revised
to incorporate form DHS-4000 (Authorization to Disclose Health Information) to
be used instead of form DHS-81 (Consent for Release of Information).
