Summary of Changes
MS 12505 #4 has been revised deleting the clinical service, annual post sterilization follow-up visit. It is no longer a covered service under the Family Planning Waiver.
MS 12515 has been updated to incorporate the procedures for the Federally Qualified Rural Health Clinics when processing family planning applications.
