How do you document eligibility determined by “informed clinical opinion” without documenting test scores?

You enter the eligibility code of “06” for developmental delay as the primary disability and enter “05T” as the secondary diagnosis. (remember, medical diagnosis must include a physician diagnosis, for this reason the 05T refers to informed clinical opinion that a child is functioning as though he/she were developmentally delayed due to specific factors) 

Do I submit data input sheets through the SEA’s program for children ages 0-3?

No, not at this time.  When the SEA’s program has been implemented fully for children ages 3-5, the agency will review and make a determination for the 0-3 program.
