The Basics:

Arkansas Outline Flag Bullet Arkansas was one of 10 states awarded funds from the Centers for Medicare and Medicaid Services to improve home and community based services.

Arkansas Outline Flag Bullet The grants are part of the “New Freedom Initiative,” which promotes the goal of community living for individuals with disabilities and long-term illnesses. Under this initiative, which began in 2001, 10 federal agencies work with states and other entities to remove barriers to community living.

Arkansas Outline Flag Bullet The Systems Transformation Grant:

  • $2, 856, 575 Award
  • 60 months  (October 2005 – September 2010)
  • Nine months planning phase
  • A collaborative effort with consumers, stakeholders, the Division of Aging and Adult Services, the Division of Medical Services, and the Division of Developmental Disabilities

The Goals:

Arkansas Outline Flag Bullet Develop and implement a statewide one stop system for long term care services and options

Arkansas Outline Flag Bullet Develop and implement a comprehensive quality management strategy, consistent with Arkansas' transformation of its long-term support system.

Arkansas Outline Flag Bullet Transform Information Technology to support systems change that includes applications that support program practices and processes that are individual-centered and enable persons to direct their own services.

Arkansas Outline Flag Bullet Develop and implement AR SOURCE, a system of primary care case management targeting dually eligible persons and others with chronic illnesses.

The Strategic Plan:

The strategic plan for the grant is to be developed within the first 9 months of the grant period.  Systems transformation within the state is a complex undertaking and the process involves principle players at a high level within state government as well as other key stakeholders, advocates, and consumers.

To ensure a comprehensive and inclusive process for strategic planning, a Systems Transformation Steering Committee of individuals composed of consumers, advocates, interested stakeholders and state staff met January 23-24, 2006.  During the two-day meeting, the team participated in the decision-making process regarding the four goals and objectives of the grant.

 A subgroup of the team met February 22-23, 2006, to develop mission and vision statements and the strategic plan.   Strategies, action steps, outputs and outcomes were discussed and defined.   Once completed, the written plan will be sent to each member of the Strategic Planning Team for final review and posted on this website.

CMS established a timeline for the Strategic Plan and the final draft which was submitted to the Center for Medicare and Medicaid (CMS).
