DDSSA recruits Health Care Professionals to perform consultative examinations and provide us with a report of their objective findings.  Social Security requires that we obtain reports from “acceptable medical sources.”
Acceptable medical sources are individuals who have the training and expertise to provide us with the signs and laboratory findings based on medically acceptable clinical and laboratory diagnostic techniques that establish the existence of a medically determinable physical or mental impairment.  Licensed or certified school psychologists, or licensed or certified individuals with other titles who perform the same function as a school psychologist in a school setting, are acceptable medical sources for establishing the existence of mental retardation, learning disabilities, and borderline intellectual functioning.  Licensed podiatrists are acceptable medical sources for impairments of the foot and ankle.  Qualified speech-language pathologists who are licensed by the State professional licensing agency, or are fully certified by their State’s education agency, or who hold a Certificate of Clinical Competence for the American Speech-Language-Hearing Association are acceptable medical sources.

If you are in an accepted medical source interested in accepting referrals on disability applicants, please complete the Professional Qualifications form and submit it to our office. Click here to view the form in PDF format.

Many of the forms as well as other pieces of   information offered on this site  are provided in Adobe Acrobat (pdf) format. If you do not have Adobe Acrobat Reader, you can download it for FREE! Just Click the Icon or one of the links above.
