How to Apply
You can download the necessary forms to apply for Quality Approval accreditation from this website. For assistance in completing the forms, contact your regional QA Specialist shown in the list below.
When your application is complete, mail it to your QA Specialist for review. Once the application has been favorably reviewed, it will be assigned to an Accessor who will conduct the on-site review. The Accessor will contact you and give a time frame for the review. The site reviews are conducted by QA Specialists or an Accessor from ASU Childhood Services depending upon the type of program you operate or your region.
The Division of Childcare will make the determination of your QA status and issue your certificate.
