Thanks to the addition of a new format these reports and application forms can be completed on-line and then printed. Completed forms still must be submitted to the Department physically (by mail or personal delivery) and not electronically. Access the Department’s home page at http://www.arkansas.gov/bank/ to take advantage of this new feature. Click on the link to Application & Other Forms. At this point, you will be directed to a Web page on which you can choose either of two preparation formats. Text Box: The new format is Microsoft Word. A form in this format can be completed on your computer if you have Microsoft Word 2000, or a more recent version. The second format, Adobe Acrobat, is the original format offered on the Department’s Web site. This option requires the use of Acrobat Reader software, which can be obtained free of charge. In this format, you cannot complete forms on your computer. A form must be printed and then completed. After you click on the link for your preferred format, you will be routed to a page of links to the application, reporting and miscellaneous forms on the Department’s Web site. Once you find the form you plan to submit, click on the link to that form. The form will be ready to be completed on your computer or printed and then completed, depending upon the format you selected.
