Retail Pharmacies can renew online here.
Your Arkansas Retail Pharmacy license will expire on December 31, 2009. The renewal fee is $300.
If you do not renew your license by February 1, 2010 - a $20 late fee will be added.
If you do not renew your license by March 1, 2010 - an additional $20 late fee will be added.
If you do not renew your license by April 1, 2010 - your license will be cancelled.
When renewing online:
- Enter your facility license number (AR##### or OS#####) and your facility ID number. Your license number and facility ID number are on the front of the renewal postcard that was sent to you.
- Have a credit, debit card, or check available. We accept MasterCard, Visa, and American Express. We also accept e-checks, all you need is a preprinted check.
- Print your confirmation statement and keep it with your records. If there are any problems with your renewal, this information will be needed to help resolve any issues.
- Your physical address can not be updated online. If your physical address has changed, please send something in writing to the Board letting us know the old address and the new address. Please also include any changes to mailing addresses, phone numbers, fax numbers, and email addresses.
- If your facility has undergone a change of ownership, you will still need to renew online, but you will also have to submit a change of ownership application. This is the same application that you used to apply for initial licensure.

If you do not receive your Retail Pharmacy license in the mail, do not assume that your license has been renewed. Please contact the Board at 501-682-0190 or at asbp@arkansas.gov to verify that you have an active license. |
