When renewing online:
-
Enter your pharmacist license number and the last four digits of your social security number. Your license number is on the front of the renewal postcard that was sent to you. Be sure to enter “PD” and a 5 digit number. If necessary, enter a “0” before your number.
-
Have a credit, debit card, or check available. We accept MasterCard, Visa, and American Express. We also accept e-checks, all you need is a preprinted check.
-
Print your confirmation statement and keep it with your records. If there are any problems with your renewal, this information will be needed to help resolve any issues.
-
Please note: your mailing address is public information. You can use a PO Box or your employment address as your mailing address, or your home address. Mailing lists, which are available for purchase, will contain your mailing address.
-
Your physical address is your actual home address. Your physical address will NOT be available to the public unless it is part of a freedom of information request.
-
If you have email, please enter your email address. Email addresses are not published.
-
You will need to enter at least 30 hours of CE to renew. If you enter fewer hours, you will not be allowed to complete the process. If you are a 2008 or 2009 graduate, if you reciprocated/reinstated in February, June, or October 2009 or if you are “inactive” for CE, please use the specific entries listed in the table on the CE tab to renew. All licensed pharmacists will be subject to a random CE audit.
-
If you currently hold an “Authority to Administer Immunization Certification”, you will be prompted to upload your CPR card on the CE tab. The link to upload your CPR card can be found at the top of the CE tab as “Immunization”. You can also mail, email or fax this information to the Board at:
- Email: asbp@arkansas.gov
- Fax: 501-682-0195
- Mail: Arkansas State Board of Pharmacy
101 E. Capitol, Suite 218
Little Rock, AR 72201
If you choose to email, fax, or mail your CPR card you must include your license number on your submittal in order to have your Certification renewed.
If you do not receive your Pharmacist license or your Authority to Administer Immunization Certification in the mail, do not assume that your license has been renewed. Please contact the Board at 501-682-0190 or at asbp@arkansas.gov to verify that you have an active license. |
