Note:  Credit/Debit card information submitted to the office will be destroyed after processing.

Reporting Period:  May 1, 2007 - June 30, 2007

Active                       $125
Voluntary Inactive     $75
Retired                      $0

Late Fee                    $100

License renewal forms and fees for fiscal year 2007-2008 in the amount of $125 are due by June 30, 2007.  Current licenses expire June 30, 2007 and will not be valid after this date until all appropriate fees and forms have been received.

Chapter 97, §17-97-204, states that any renewals post-marked after June 30, 2007 will be considered delinquent and the licensee will be charged a late fee of $100 in addition to the annual renewal fee.

If necessary, you can renew your license by mail with a credit/debit card or check/money order using the forms below.  

Psychological Examiners:  Supervision report forms must be mailed to the Board office.
